Our staff looks forward to meeting you and discussing your dental health goals. We strive to keep our appointments on time, as scheduled. Prior to your appointment, we encourage you to fill out the patient forms that are located on this website so that you can accelerate your check-in process. Please download, print and complete the forms and bring them with you to your appointment. If you are unable to download our patient forms, then it is recommended that you arrive to your appointment between 15 – 30 minutes early so that you can have ample time to fill out the appropriate forms.
Pre-Operative Instructions
New or returning after 3+ years? Print and complete all forms before your visit. Bring a med list. Minors must be with a parent/guardian. See you soon!
Post-Operative Instructions
Follow bleeding, swelling & hygiene steps. No straws, smoking, or heavy activity. Call if bleeding, fever, or swelling worsens after day 3.
New Patient Registration
Print and complete all forms. Bring them, a med list, and ID to your appointment. Minors must come with a parent/guardian.
Financial Acknowledgement
Patients are financially responsible for all fees. Payment is due at service. Insurance is not guaranteed.
Insurance Waiver
