First Visit

Initial Appointment - Cocoa Office

Your initial appointment will consist of a consultation explaining your diagnosis, radiographs (if needed), and treatment options with fee estimates. For minor procedures, surgery may be done the same day as the consultation. If you desire sedation or IV anesthesia, your first visit will be for consultation only and the surgical procedure will be done on another day.

• Your surgical referral slip from your general dentist as well as any x-rays recently taken.
• A list of medications you are presently taking.
• Medical and/or dental insurance identification cards.

IMPORTANT: All patients under the age of 18 years of age must be accompanied by a parent or legal guardian at the consultation visit.

A pre-operative examination is mandatory for patients undergoing I.V. anesthesia for surgery. Your surgery will then be scheduled at the next available appointment. Please alert the office if you have a medical condition that may require your physician's medical clearance prior to surgery (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc. or if you are on any medication (i.e. heart medications, aspirin, blood thinners, etc.).
